Conveying a sense of being in between:
Gender spectrum to show futility of gender binary. Gender spectrum could also be represented less literally, using a color spectrum. Rainbows would probably be a bit obvious for this message, but I like the idea of a "gray area" because it conveys both a visual and metaphorical meaning.
Flip up elements that has binary presentation on outside but true more androgynous self below flap (might be a bit literal and probably doesn't belong here but didn't want to lose it)
Off of that point, if I did the little people like here, I could have little flippies that show how they truly identify with the classic Venus/Mars symbols. Still seems literal, but might lead to something else later on.
Swirly inspiration on Peter Pan page: Could have an image of recognizably feminine silhouette and a recognizably masculine silhouette with their swirlies starting to blend together, and then have an androgynous figure in the negative space. Definitely want to incorporate this.
Reverse sides of accordion book: If one side is medical, and the other is fantasy, then them coming together would be an image of reality. Also, it would be interesting to have the sides mirror each other somewhat literally, with a fantastical version of an element on one side and a medical/scientific version on the reverse.
Still hanging on to the idea of people being perceived as monsters when nonbinary: Could do the gender spectrum thingie where the middle ones are monsters but then the flippy thing happens and they're just their cute little selves. I also still like the idea of portraying the gaze, but that might not work with this more narrowed focus.
